You asked

I understand that Transport for London has an “electronic fatigue reporting form” which drivers are encouraged to use in order to report "occurrences of fatigue". These reports are centrally monitored by the Safety, Health, and Environment Fatigue Team. • Please could I request a skeleton template of this fatigue reporting form. For example, what questions are asked, drop down menus, checklists, etc. • Please could I request the number of occurrences of fatigue made using this online system for the years 2020, 2021 and 2022 (between 01/01/22 and the day this request is processed). If possible, please break this data down by vehicle type - i.e., train, tube and bus.

Thank you for your request received on 22 November 2022 asking for information about fatigue reporting.

Your request has been considered in accordance with the requirements of the Freedom of Information Act and our information access policy. I can confirm we hold some of the information you require. You asked:

•           Please could I request a skeleton template of this fatigue reporting form. For example, what questions are asked, drop down menus, checklists, etc.

Please find attached  a copy of the form. The reporting form is applicable to our directly recruited staff only. 

•           Please could I request the number of occurrences of fatigue made using this online system for the years 2020, 2021 and 2022 (between 01/01/22 and the day this request is processed). If possible, please break this data down by vehicle type - i.e., train, tube and bus.

The online process was launched in September 2021 and was accompanied by a series of online workshops and drop in sessions to support our front line managers. The first online Fatigue Reporting Form was first submitted in September 2021 and the table below breaks down the numbers of reports received as per your request. Please note that these are reports for the London Underground only.

Separately, bus drivers are employed by the companies they work for and operate their own fatigue management system and therefore we do not have a centralised database for reporting from bus drivers. Serious incidents involving buses where fatigue is suspected to have been a factor are subjected to a thorough investigation to establish root causes and corrective actions. However, at present the findings of these individual investigations are not recorded in a centralised database.

We continue to work across TfL to help reduce the instances and impacts of fatigue and are wholly committed to fostering a just and fair culture where colleagues feel comfortable reporting fatigue.
