F/on from FOI-4767-3234/GH Thank you for your response of 22nd April to my FOI request. From this, I have been advised to request the further information. 1. Please provide a copy of your highway maintenance policy, which includes details of all inspections that should be carried out, the frequency of inspections, how the inspections should be carried out and the competency requirements for the inspectors, as well as what records should be completed. 2. Please provide a copy of your highway maintenance policy, which includes details of what maintenance should be carried out, the frequency of maintenance, who carries out the maintenance and what records are generated by the works. Specifically for the previously reported defect, please provide the following information: a. Please detail what asset the man hole cover relates to - e.g. drainage / communications etc. b. When was the carriageway section last inspected for each category of required inspection. c. What were the findings of the inspections and what records are available. d. When was the chamber last inspected. e. How were all of the types of inspections undertaken. f. Who undertook the inspections, including a summary of their training and accreditations. g. When was routine maintenance of the chamber last undertaken. h. Please provide records of the last round of maintenance. i. Were any defects previously reported in the vicinity and what was the response.

Thank you for your follow up request to TfL ref 4767-2324 request received by Transport for London (TfL) on 3 August 2024 asking for information about a Pothole on the A12.

Your request has been considered in accordance with the requirements of the Freedom of Information Act and our information access policy.  I can confirm that we hold the information you require. Your specific questions and our replies are as follows:
 
  1. Please provide a copy of your highway maintenance policy, which includes details of all inspections that should be carried out, the frequency of inspections, how the inspections should be carried out and the competency requirements for the inspectors, as well as what records should be completed.
  2. Please provide a copy of your highway maintenance policy, which includes details of what maintenance should be carried out, the frequency of maintenance, who carries out the maintenance and what records are generated by the works.

Specifically for the previously reported defect, please provide the following information:
 
  1. Please detail what asset the man hole cover relates to - e.g. drainage / communications etc.  – This asset is a drainage manhole and relates to  Inspection Chambers - 8140328
  2. When was the carriageway section last inspected for each category of required inspection –  The inspection report is attached covering the dates requested and to current date.
  3. What were the findings of the inspections and what records are available. – There were no defects recorded against this asset or the related carriageway asset prior to the manhole becoming defective. Attached is the Defects and Work Orders Report going back from 09/03/24 to current date.
  4. When was the chamber last inspected?  - The inspection report is attached covering the dates requested and to current date.
  5. How were all of the types of inspections undertaken. – The inspections are carried out by TfL’s designated Works for London contractors on a weekly basis as a driven visual inspection exercise formerly agreed by TfL.
  6. Who undertook the inspections, including a summary of their training and accreditations. - The inspections are carried out by our contractors’ Inspectors who hold the City & Guilds 6033 - Unit 311 qualification.
 
  1. When was routine maintenance of the chamber last undertaken.
  2. Please provide records of the last round of maintenance.
  3. Were any defects previously reported in the vicinity and what was the response.

Response to g) h) and i):
All routes within the TfL network (TLRN) are inspected by our safety inspectors on a weekly basis (see Qu e above). Should any defects be noted they will be raised dependant on severity and if the asset is deemed dangerous it will be prioritised to be resolved within the hour.
