i would also like to request the health and safety assessment for northern line trains, and the accident reporting procedures for bones broken on the underground.

Please find attached a copy of the risk assessment for the Northern line.

London Underground requires that all accidents, incidents and near misses are reported and investigated. Where it is confirmed that a customer has a fracture (other than to fingers, thumbs and toes as per the Reporting of Injuries, Diseases & Dangerous Occurrences Regulations 2013) a detailed fact find called a ‘Go, Look, See’ is carried out by the local management team. This enables the circumstances of individual incidents to be understood by attending the site of the incident as soon as possible after it has occurred. It would include (and not be limited to) a review of the incident report made at the time, a review of the CCTV and a review of the customer risk assessment depending on the nature of the incident. Based on the findings recommendations would be made as needed to look at how similar incidents could be prevented in the future. All incidents notifiable under RIDDOR are also reported to our regulator (the Office of Rail and Road).
