(i) please confirm who approved the criteria and when, and

Your request has been considered in accordance with the requirements of the Freedom of Information (FOI) Act and our information access policy. I can confirm TfL does hold the information you require. You asked:
(i) please confirm who approved the criteria and when, and (ii) please provide an electronic copy of the criteria.
The criteria you refer to is the TfL Penalty Fare Appeals Policy available on the TfL website. It was approved by Siwan Hayward, Director of Compliance, Policing, Operations & Security TfL on 1 Nov 2021.
