Under the Freedom of Information Act, I am requesting data relating to coroners' Prevention of Future Deaths reports (PFDs or Reg 28 letters) received by Transport for London. Can I please request the following: 1. the total number of PFDs where Transport for London was a listed addressee from inception (July 2013) until April 2024 (or the most recent available data), broken down as annual counts for each calendar year (e.g. for 2013 the number of reports from Jul to Dec; for 2014 the number of reports from January to December). 2. the total number of PFDs where Transport for London was copied for information in as an ‘interested person’ from inception (July 2013) until April 2024 (or the most recent available data), broken down as annual counts for each calendar year (e.g. for 2013 the number of reports from Jul to Dec; for 2014 the number of reports from January to December).

Thank you for your request received on 16 May 2024 asking for information about coroners’ Prevention of Future Death reports.

Your request has been considered in accordance with the requirements of the Freedom of Information (FOI) Act and our information access policy. I can confirm that we hold some of the information you require.

Safety is both the Mayor and TfL’s priority and our customers, users, employees and suppliers have an expectation that when they are using or delivering our services or assets they will remain harm free. We undertake investigations into incidents when they occur to ensure we learn lessons from any tragic incident and continue to improve the safety of our services. We also implement any recommendations that might be made by any regulatory authority that carries out an investigation. Prevention of Future Death reports issued to TfL are reviewed and considered and relevant actions taken as part of our continuing focus on achieving Vision Zero and eliminating all deaths and serious injuries from our network.

You asked for:

1.         The total number of PFDs where Transport for London was a listed addressee from inception (July 2013) until April 2024 (or the most recent available data), broken down as annual counts for each calendar year (e.g. for 2013 the number of reports from Jul to Dec; for 2014 the number of reports from January to December).

2.         The total number of PFDs where Transport for London was copied for information in as an ‘interested person’ from inception (July 2013) until April 2024 (or the most recent available data), broken down as annual counts for each calendar year (e.g. for 2013 the number of reports from Jul to Dec; for 2014 the number of reports from January to December).
