Whilst we believe we have provided all incidents in relation to your request, it’s not possible to know definitively whether the list of fatal investigation reports includes all incidents, as there may have been some where the investigation hadn’t been closed at the time it was compiled, and therefore don’t feature on the list. We would likely need to conduct a manual task of cross referencing this with the incident database to determine whether we captured all relevant incidents.
Unfortunately carrying out this exercise would exceed the ‘appropriate limit’ of £450 set by the Freedom of Information (Appropriate Limit and Fees) Regulations 2004. Under section 12 of the FOI Act, we are not obliged to comply with a request if we estimate that the cost of determining whether we hold the information, and then locating, retrieving or extracting it from other information, would exceed the appropriate limit. This is calculated at £25 per hour for every hour spent on the activities described – equivalent to 18 hours work.
If this is not the information you are looking for, or if you are unable to access it for any reason, please do not hesitate to contact me.
Please see the attached information sheet for details of your right to appeal as well as information on copyright and what to do if you would like to re-use any of the information we have disclosed.
