I have been going through the documents which has been very useful, many thanks.
One document, "KPMG Chronology of Events", https://tfl.gov.uk/cdn/static/cms/documents/kpmg-chronology-of-events.pdf, refers to:
A Lessons Learned report entitled “Programme Partner Independent Review into Lessons Learned during Stage 1 Testing and Commissioning and Bringing into Use” was issued in draft and dated 27 July 2017.

I cannot find this document on the site you provided the link to, how do I obtain a copy? I am accessing these documents for some research into project failures and I suspect a lot of useful information will be available in all of there LEssons Learned documents or documents that have the same purpose but may ave a different title, "Project (or Performance) Review" for example.
I tried using the search function for the site but it searched for each word rather than the term and produced 184 pages of responses.
