Please provide a complete list of all reasons PHV Operator licenses were refused to be renewed during the month of September, and whether any appeal has been lodged against any of those reasons or any decision - whether or not those cases are yet to be heard by any court or panel. If any appeal has been lodged, please specify the date on which the appeal was received, and by which court or panel.
