SCHEDULE 5 - Disruption, Defects and Failures

Part A - Disruption, Defect, and Failure Definitions
The Disruption, Defect, and Failure types referred to in Schedules 3 ( Reporting Procedures ) and 4 ( Fault Attribution Procedures ) are defined in the tables below.

Train Delays


Train Service Availability


Where a Train stops for a period of time longer than scheduled at any specific location as a direct consequence of an Incident.


The Train Delay shall be logged against one Train only, even though other Train(s) may be impacted.

The Initial Delay may be in the form of an unscheduled stop, or it may be an unduly extended stop that was due to have occurred.

The Initial Delay must be two (2) minutes or more in duration.


Signal Failures


Train Service Availability


Where a Train is delayed, followed by a period of through-running at reduced speed affecting subsequent Trains which is due to a signal, points or track circuit Asset Failure or Defect until such signal is fully restored.


The Signal Failure shall be logged against one Train only, even though other Train(s) may be impacted.

The Signal Failure must be two (2) minutes or more in duration.

Where a Signal Failure experiences a subsequent Initial Delay ( possibly due to additional investigatory or repair work ), the Signal Failure is considered to have ended and a new Signal Failure shall be logged from the start of the new Initial Delay.

Signal Failures spanning more than one (1) day shall be logged separately on each day.


Train Withdrawals


Train Service Availability


Where all Customers are instructed to vacate the Train and the Train is withdrawn from scheduled Customer service.


The withdrawn Train may or may not be replaced.

The withdrawn Train may be replaced or reinstated immediately, in which case no further Lost Customer Hours shall be incurred.

The withdrawn Train may be replaced or reinstated at some later point in time, or not at all, in which case some Lost Customer Hours shall be incurred.

The Train Withdrawal must be two (2) minutes or more in duration.


Train Cancellations


Train Service Availability


Where a Train does not enter scheduled service from a stabling location, resulting in Lost Customer Hours.


The cancelled Train may be replaced or reinstated at some point during its scheduled period of operation ( but at some time later than the scheduled commencement of service provision ) in which case no further Lost Customer Hours shall be incurred.

Multiple cancellations shall be logged as separate individual Train Cancellations except where a Speed Restriction is the primary cause when multiple Train Cancellations will be treated as a single incident.

Train Cancellations resulting from the withdrawal of depot services during the day shall be logged as individual Train Cancellations. Where depot facilities are unavailable as a result of a Minor Closure and the Lost Customer Hours are to be calculated from the resulting Train Cancellations ( i.e. the depot facilities have become unavailable during the course of the traffic day ), Train Cancellations will be applied to all directly affected Trains scheduled to stable at the affected depot and the calculation of their cancellation durations will take account of each affected scheduled trip and their preceding trips ( i.e. at least one whole round trip per affected train to account for the requirement to stable the Train early at the affected depot. )


Depot Late Start-ups


Train Service Availability


Where Trains are unable to enter or leave scheduled service from depots, sidings and other stabling locations including platforms.


The Depot Late Start-up shall be logged from the commencement of services in the morning.

Train Cancellations arising from this type of disruption should not be logged as separate Disruptions.

A Depot unavailable during the operating day shall be recorded as individual Train Cancellations.

Depots can be considered late starting or unavailable if, as a consequence of a Partial Line Suspension, depots are affected such that Trains are unable to enter service and serve the LUL zone one area, or return to depot having served the LUL zone one area on their normal routes through the central area.

Depots serving more than one Line shall be logged separately for each Line.

The Depot Late Start-up must be two (2) minutes or more in duration.


Partial Line Suspensions


Train Service Availability


Where no Train Service is Available between specific Stations on the Line.


The Partial Line Suspension must be two (2) minutes or more in duration.

(a) Depots can be considered late starting or unavailable if, as a consequence of a Partial Line Suspension, depots are affected such that Trains are unable to enter service and serve the zone one area, or return to depot having served the zone one area on their normal routes through the central area.

(b) Where depot facilities are unavailable as a result of a Minor Closure the Lost Customer Hours shall be determined as being the greater of either the Partial Line Suspension or the impact upon the depot. For the avoidance of doubt, this means:








(i) where the depot facilities are unavailable from the start of traffic ( e.g. weekend closures ) the Lost Customer Hours shall be the maximum arising from either of the disruption types Partial Line Suspension or Depot Late Start;

(ii) where the depot facilities become unavailable during the traffic day ( e.g. early shutdowns ), the Lost Customer Hours shall be the maximum arising from either of the disruption types Partial Line Suspension or Train Cancellations.








(c) Notwithstanding sub-paragraph b ) above, disruption type Partial Line Suspensions will apply when calculating Lost Customer Hours for a Saturday, Sunday, Weekend or Bank Holiday Minor Closure, ( but only where depot facilities are unavailable from the start of traffic for the entire day or weekend period as the case may be ) provided that:

(i) Infraco uses its best endeavours to provide those Trains requested by LUL to enable LUL to operate its preferred alternative service; and

(ii) in any event Infraco provides the Minimum Fallback Train Requirement for central area service provision.








(d) In determining whether or not Infraco has provided the Minimum Fallback Train Requirement, LUL shall act reasonably and may, if it considers it reasonable and practicable to do so:

(i) accept train provisions from depots and stabling locations that may differ from normal operations; and

(ii) agree to stabling of Trains at locations that may differ from those used during normal operations.


Full Line Suspensions


Train Service Availability


Where no Train Service is Available throughout the Line in either direction.


The Full Line Suspension must be fifteen (15) minutes or more in duration.


Speed Restrictions


Train Service Availability


Where an agreed Speed Restriction is in force, resulting in extra running time between two (2) adjacent Stations.


Train Cancellations arising from Speed Restrictions shall not be logged separately.

Speed Restrictions in both directions must be logged separately.

Speed Restrictions spanning more than two (2) adjacent Stations should be logged as a series of Speed Restrictions covering the adjacent Station Pairs.


Escalator Downtimes


Lift, escalator and Customer conveyor Availability


Where one or more escalators is either taken out of service completely or used as a fixed stairway.


Escalator Downtime refers to a single escalator which may be one on its own, or part of a bank of escalators. Where more than one escalator is out of service in a bank of escalators, each escalator which is out of service must be recorded separately.

If the escalator was out of service from the start of the operating day ( i.e. it was not turned on ), then Station opening time shall be deemed as the time of commencement of Escalator Downtime.








The Escalator Downtime must be two (2) minutes or more in duration. The Escalator Downtime may last for several days ( or weeks ) whilst repair works are being undertaken.


Lift Downtimes


Lift, escalator and Customer conveyor Availability


Where one or more lifts is taken out of service completely.


Lift Downtime refers only to a single lift which may be one on its own, or one of a bank of lifts. Where more than one lift is out of service in a bank of lifts, each lift which is out of service must be recorded separately.

If the lift was out of service from the start of the operating day ( i.e. it was not turned on ), then Station opening time shall be deemed as the time of commencement of Lift Downtime.

The Lift Downtime must be two (2) minutes or more in duration. The Lift Downtime may last for several days ( or weeks ) whilst repair works are being undertaken.


Customer Conveyor Downtimes


Lift, escalator and Customer conveyor Availability


Where one or more Customer conveyors is either taken out of service completely or used as a fixed walkway.


Customer Conveyor Downtime refers only to a single Customer conveyor which may be one on its own, or part of a bank of Customer conveyors. Where more than one Customer conveyor is out of service in a bank, each Customer conveyor which is separately out of service must be recorded.

If the Customer conveyor was out of service from the start of the operating day ( i.e. it was not turned on ), then Station opening time shall be deemed as the time of commencement of Customer Conveyor Downtime.

The Customer Conveyor Downtime must be two (2) minutes or more in duration. The Customer Conveyor Downtime may last for several days ( or weeks ) whilst repair works are being undertaken.


Platform Closures


Station Service Availability


Where one or more platform is not Available.


The Platform Closure must be two (2) minutes or more in duration.


Full Station Closures


Station Service Availability


Where all Station services are suspended, including entry, exit and interchange.


The Full Station Closure must be two (2) minutes or more in duration.


Partial Station Closures


Station Service Availability


Where one or more Ticket Halls, Ticket Office, Routeways or Station Entrances are unavailable in accordance with the categories set out in paragraph 4.4.6 of Section Two of this Performance Measurement Code.


The Partial Station Closure must be two (2) minutes or more in duration.

Where one or more such Assets are unavailable as a result of a single Incident, the unavailability of such Assets shall comprise a single Partial Station Closure.

Part B - Data Requirements

1. Service Points and NACHs values are calculated based on the data requirements for each type as set in the tables below. The calculations performed are detailed in paragraphs 4 and 6 of Section Two of this Performance Measurement Code.

2. Where LUL decides to withdraw an Asset pending independent investigation, then the End of Initial Delay ( and/or Duration where appropriate ) is considered to be the time the Asset is withdrawn.

3. Where supplementary information has been obtained during the course of the fault attribution process, this data shall be entered into the supplementary information field in CuPID.

4. Where an Incident has been accorded the status of No defect Found during the fault attribution process, this shall be recorded in the NdF flag field in CuPID or the FRC database.

5. Where a fault is expected to result in safety or environmental contravention orders being issued against London Underground by the Health and Safety Executive ( including HMRI ), the London Fire and Civil Defence Authority, Local Councils or any other regulatory body, it shall be recorded as a contravention in the FRC database.

Length and position of Speed Restrictions

The length of track measured in metres to the nearest metre between the closest preceding platform stopmark relative to the direction of travel and the start of the speed restriction. The platform shall also be identified.

The length of track to which the Speed Restriction is applied, measured in metres to the nearest metre. The length of track is defined as the distance between the start of a speed restriction, as defined by the initial sign showing the maximum permitted speed, and the end of the speed restriction, as defined by the 'T' indicator sign.

Part C - Codes and Formats used
The data requirements listed in the tables in Part B of this Schedule are subject to the following codes and format rules:

1. Unique Reference Numbers

1.1 There are two (2) types of unique reference number: CuPID Incident Number and FRC Number. When a data record is entered into either of the two (2) systems ( CuPID or FRC Database ), the record shall be assigned a unique reference number within the relevant system. The formats are not described here.
1.2 For Incidents recorded in CuPID, a CuPID Incident Number is also assigned. Where multiple Disruptions are caused by the same Incident, the CuPID Incident Number applies to all Disruptions caused by that Incident.
2. F&D Number

The F&D Number is the unique reference number given to an Incident or Disruption by the Line Controller when the Incident or Disruption details are recorded in the F&D. The format is not described here. The F&D Number is entered into the CuPID. In these cases, where the FRC has also been contacted to investigate and carry out necessary repair work or corrective action, the F&D Number shall also be entered into the FRC Database and subsequently transferred into the CuPID.

3. Line

One of the following name codes shall be recorded in respect of the corresponding Line:

4. Location
The locations used shall be recorded such that the Stations which are named are clear and unambiguous.

The following rules shall apply to Train Service Disruptions: (a) Where the Train is inside or partially inside a Station, then the Station name shall be recorded.

(b) Where the Train is between two (2) Stations but has not reached the next Home Signal, then the preceding Station, being the Station that has been passed, shall be recorded.

(c) Where the Train is between two (2) Stations and has passed the Home Signal, then the Station being approached shall be recorded.

5. Direction

The directions used shall be recorded using the following codes:

8. Dates and Times
All dates and times recorded shall be clear and unambiguous including: (a) Start Time;

(b) Time of Notification;

(c) End of Initial Delay;

(d) End of Duration;

(e) Transfer In Time;

(f) Release Time; and

(g) Extended Clearance Time.

9. Continuation Flag
